Transition to Kindergarten

Register Your Child for Kindergarten in Centre County Schools - 2009-2010 School Year

** September 1, 2009 for Bellefonte Area School District.

Proof of birth date (birth certificate, baptismal certificate if birth date is shown, or passport). You must bring the original document. Photocopies will not be accepted.
Guardianship & Custody Orders. Persons with legal guardianship or legal custody of a child must bring copies of the documents granting the guardianship or custody.
Proof of residency in the school district (deed, lease, or current utility bill for place of residence)
Immunization Records. Bring a list of your child's immunizations and the dates given – it can be a list from your doctor or a record that you have kept.

Immunizations can be obtained from your family physician or from the State Health Center, 280 W. Hamilton Ave., State College, PA 16801. Call 865-0932 between 8:00 a.m. and 4:00 p.m., Monday to Friday, to make an appointment at the Health Center and bring all prior immunization records for the child to the Health Center.

Physical Exam
Pennsylvania School Health Law requires a physical exam for all Kindergarten students. The exam is not required for registration. The exam must be done no more than 12 months prior to the start of school and may be done during the school year. The exam can be given by your family physician or provided by the District at no cost. Additional information will be provided at registration.
